Dental Care:

Services such as check-ups, x-rays, cleanings, tooth-whitening, crowns, dentures and fillings are discounted to Members by as much as 85% off Usual & Customary fees. This can save you and your family $100s each year - just look at these sample savings:

Vision Care:

Our vision care providers agree to discount eye exams by $5 to $10, eyeware by as much as 50% and laser surgery by as much 25%.  Here is our fee schedule:

Exam (with Dilation as necessary):

• $10 off routine exam
• $15 off contact lens exam

Frames:

• Up to $130: 30% off retail price
• Over $130: 20% off the balance

Standard Plastic Lenses*:

• Single: $55 or less
• Bifocal: $75 or less
• Trifocal: $95 or less
• Lenticular: $95 or less
• Standard Progressive: $125 or less

Lens Options (add to base price of lenses):

• UV Coating: $12.50
• Tint (Solid and Gradient): $12.50

What does Usual & Customary mean?

Usual & Customary (U&C) is an industry term. It basically means: “what constitutes a fair price for quality medical and dental services in a particular l ocation.” 

U&C Fee schedules are often established by Managed Care Companies to determine how they reimburse doctors. They help contain costs (because the insurance company is responsible for payment), while assuring that providers treat their patients diligently and thoroughly to the best of their ability.  U&C Fee schedules also take into account the fact that underlying costs for doctors vary from community to community.  Therefore, U&C in Miami may not be the same as in Palm Beach.

Care4Neighbors uses fee schedules and contracted terms to set our Providers’ fees below U&C. We determine U&C in much the same way that managed care companies do, always with the goal of allowing our Members an honest discount and our Providers with a fair price for quality work.   

Care4Neighbors is not insurance. This is a Discount Plan. You are responsible for payment to your healthcare professional, pharmacy or lab of the entire costs of services. Those services have been discounted to our Members, however, Care4Neighbors does not reimburse you or your doctors for any charges you incur in the course of your treatment.
